Where Will Unify Care Providers Visit Me?
We will visit you at home, work, hospital, rehab, or skilled nursing facility within Orange County, Riverside, and Palm Springs. If we do not have privileges at your facility, we will still visit you to advocate for you, and coordinate your care with your care team. Rest assured that if you need us, we will find a way.
Can I Contact My Doctor Directly?
Yes, our model ensures that you can directly contact your care team via secure messaging or phone, enhancing your access to personalized care.
What Types Of Healthcare Services Do You Provide?
We provide a wide range of services from primary care to specialized services, tailored longevity plans, anti-aging treatments, chronic disease management, and preventive healthcare.
How Much Does Unify Care Membership Cost?
Our monthly membership options are Unify Standard at $499 and Unify Premium at $999. We also have discounted annual membership options.
Can I See a Doctor If I’m Not a Member?
Yes, our Unify Pay-Per-Visit plan is designed for non-members who need one-time consultations or who are seeking a second opinion. You can always upgrade to a membership after your visit but this is not required.
What Happens If I Need To See a Specialist?
We provide expedited referrals and coordination with specialists. Our care team will ensure you receive timely and seamless care across all your healthcare needs.
How Do You Handle After-Hours Emergencies?
Premium members have 24/7 access to Telehealth consultations. For true emergencies, you will need to dial 911. We are happy to help coordinate your care by calling Urgent Care or the Emergency Room.
How Do Lab Tests and Prescriptions Work?
We order all lab tests and prescriptions for you.
Can I Use Insurance?
Insurance reimbursement to the patient varies, depending on your insurance and services provided.
Can I Use My FSA or HSA To Pay For My Membership?
